Description
This training provides educators with the knowledge and tools to write effective, individualized transition plans that comply with IDEA and meaningfully support students as they prepare for life after high school. Participants will explore the required components of a transition plan—including measurable postsecondary goals, transition services, and annual IEP goals—and learn how to write them in clear, student-centered language.
The session emphasizes the importance of aligning postsecondary goals with transition assessment data to ensure they reflect each student’s strengths, preferences, and interests. Educators will practice writing coordinated transition services that support movement toward those goals and connect annual IEP goals to the student’s long-term outcomes.
