Transition planning is a critical part of preparing students with disabilities for life after high school. In this session, participants will gain a clear understanding of when transition planning must begin and the legal timelines that guide the process. Attendees will learn how to define transition services and explore how these services support students in achieving postsecondary goals related to education, employment, and independent living. This session will also break down the required components of a transition plan in the IEP, including the use of age-appropriate assessments, the development of measurable postsecondary goals, and the coordination of services that reflect the student’s needs and aspirations. Real-world examples of effective transition services and student-centered activities will be shared to help educators and support staff align services with individualized goals.
