This presentation will prepare school nurses to implement a School Nurse Case Management (SNCM) Program for in their school. SNCM provides supports to students to better manage chronic health conditions and can improve student academic performance.

Additionally, SNCM is a great overview of school nurse interventions for managing chronic health conditions for new or practicing school nurses who want to improve their practice. This course will include the following topics:
